That is where an EMG test, or electromyography, can become useful. An EMG is not a test that everyone with muscle or nerve pain automatically needs. So, when should you get an EMG? The answer depends less on having one particular symptom and more on the pattern of symptoms, how long they have been present, and what a neurologist finds during an evaluation.
 

What Is an EMG?

Electromyography is a diagnostic procedure that evaluates the health and function of muscles and the nerves that control them.

During a needle EMG, a neurologist uses a small electrode placed into selected muscles to record electrical activity. The test can show changes in muscle activity that may point toward a nerve or muscle disorder.

An EMG evaluation commonly includes a nerve conduction study (NCS) as well. Instead of recording activity directly from a muscle, nerve conduction testing uses electrodes placed on the skin to measure how effectively electrical signals travel along nerves. Together, these studies can provide information about where a problem may be occurring and help a neurologist support or rule out particular diagnoses.

That distinction matters because symptoms such as numbness or weakness do not always reveal their source on their own. A person may feel pain in the hand, for example, while the underlying problem involves a nerve farther up the arm or a nerve root near the spine. When Do You Need an EMG?

There isn't a single symptom that automatically means you need an EMG.

Instead, doctors may recommend the test when symptoms suggest a problem involving the peripheral nerves, nerve roots, muscles, or the communication between nerves and muscles.

Common reasons include:

  • Persistent or unexplained numbness

  • Tingling or a "pins and needles" sensation

  • Muscle weakness

  • Certain types of limb pain

  • Muscle cramping

  • Muscle twitching or other unusual muscle activity

  • Muscle wasting or atrophy

  • Symptoms that suggest a pinched or damaged nerve

Numbness and Tingling: When Should You Get an EMG?

Numbness and tingling can have many causes, ranging from temporary pressure on a nerve to conditions affecting peripheral nerves or nerve roots.

An EMG may be considered when these sensations are persistent, unexplained, recurrent, or accompanied by other neurological symptoms.

For example, someone who repeatedly experiences tingling in the thumb, index finger, and middle finger may be evaluated for carpal tunnel syndrome. An EMG can help determine whether weakness is associated with a nerve problem, a muscle disorder, or another neuromuscular condition.

AANEM identifies weakness, including fatigable weakness, as one of the symptoms that may be evaluated with electrodiagnostic testing.

That does not mean every episode of weakness requires an EMG. The neurological examination and the overall medical history help determine whether the test is likely to provide useful information.
 

Which Conditions Can an EMG Help Diagnose?

One of the strengths of an EMG and nerve conduction study is that they can help a neurologist investigate different parts of the neuromuscular system.

Depending on the symptoms and clinical question, testing may help evaluate conditions such as:

  • Carpal tunnel syndrome

  • Peripheral neuropathy, including some forms of diabetic neuropathy

When Might an EMG Not Be Necessary?

This is an important part of the conversation because more testing is not always better testing.

What Happens During an EMG Test?

During the needle portion, the neurologist places a fine needle electrode into selected muscles and records electrical activity while the muscle is relaxed and while it contracts.

The exact muscles and nerves tested depend on the symptoms and the diagnostic question. What Can the Results Tell You?

An EMG doesn't simply produce a pass-or-fail result.

The findings are interpreted in the context of your symptoms and examination. They can help a neurologist determine whether there is evidence of nerve dysfunction, muscle dysfunction, or problems affecting communication between nerves and muscles.

The results may also help with questions about where a problem is occurring and, in some circumstances, its severity or timing.

That information can be useful when symptoms are difficult to explain from the outside. Someone may know that their hand feels weak or their foot keeps tingling, but the underlying cause isn't always obvious from the symptom alone.

So, When Should You Get an EMG?

If you have persistent numbness, tingling, unexplained weakness, certain types of nerve or limb pain, muscle cramping, or other symptoms suggesting a nerve or muscle disorder, it may be worth discussing EMG testing with a neurologist.

But the decision should be based on the pattern of symptoms and clinical evaluation, not on the presence of pain or tingling alone.

That distinction is important. An EMG is most useful when it answers a specific diagnostic question.

If your symptoms are difficult to explain, keep returning, are accompanied by weakness or other neurological changes, or suggest a problem affecting the nerves or muscles, a neurological evaluation can help determine whether electromyography and nerve conduction testing are appropriate.
